New Patient Information
Thank you for considering the Santa Monica Wellness Center for your health care needs. We consider it and honor and a privilege to be your partner in assisting you through a healing process.
People consider the Santa Monica Wellness Center for various reasons:
Some patients come who aren't quite "sick" but want to increase their well-being and learn about how to prevent illness. They are looking for optimal health and longevity.
Some reasons these patients seek care because they are:
—experiencing low energy/fatigue
—interested in learning how to prevent illness/disease
—overwhelmed with their stressful lifestyle
—looking for increased performance
—looking to age with grace and ease
—seeking longevity and feeling good—adding life to their years, not just years to their life
—looking for help with nutrition
—looking to lose weight
—curious about holistic therapies available
—looking to maintain the high degree of health and wellness they already enjoy
Existing Health Concerns
Some patients come because they already have a diagnosed condition or have symptoms that need assessment and treatment.
They may be looking for something to complement the Western treatment they are receiving, or they may be looking for a more natural approach to treating their condition.
For approaches to both Wellness and Existing Health Concern visit the Services Section where you will also see the conditions that we can offer support.
What to Expect on the First Visit
A comprehensive intake and history will be taken. This will include your physical health as well as lifestyle factors that may affect your condition.
Please bring in your completed questionnaires and other forms that were mailed or faxed to you in your new patient packet. Also bring past medical records, lists of nutritional supplements and medications you are taking, as well as those you have taken in the past for a substantial period of time.
Allow at least an hour and a half for your first visit. It is best to plan on being in the office approximately two hours. We pride ourselves on not overbooking and we generally run on time; however it is best to allow a little extra time so you have time to check in and out without rushing.
It is best to plan on being in the office for an hour for your follow up visits, if you have a basic appointment. The follow up visits generally last from 30-40 minutes, but again please allow enough time so you're not rushing and feeling stressed.
Some patients will need to book extended appointments for various reasons: out of towners, more complicated health concerns, reviewing multiple lab tests, etc. An extended follow up visit lasts between 60 and 75 minutes so plan on being in the office an hour and a half.
During the first visit, an exam and treatment will usually be included. If your history is extensive, the exam and treatment will be performed on the second visit. If you are in pain or suffering from an acute condition, the exam and treatment will be performed on the first visit. We will decide together how to make the most valuable use of the time together.
The modes of treatment can include acupuncture, acupressure, herbal, homeopathic, and nutritional therapies. Nutritional, lifestyle, and stress management counseling may also be included. In additional to providing relief from suffering, we are looking to provide long-term solutions. We seek to identify the cause of the condition as well as treating the symptoms. Our intention is to partner with you in creating optimal health.
The goal of the first visit is to establish a basic treatment plan. After assessment, we may need to order laboratory testing. If this is necessary, the lab results will be reviewed at subsequent appointments.
If your condition requires a referral or collaboration with a Western medical professional, or another health care provider, we will happily refer you. We have excellent colleagues who are leaders in their respective fields, and in whom we feel confident in referring. Our intention is to assist you in achieving your health goals. Sometimes we are the right practitioners for you, and sometimes after your initial assessment, we may realize you need to see a different type of practitioner who would better suit your health care needs.
Payment is due at the time of the visit. Payments accepted are Visa, Master Card, American Express, Discover, cash and check. We do not bill business managers, insurance companies, etc. Post-dated checks are not accepted.
We do not bill insurance. If you have insurance, payment is still due at the time of the visit. However, we will issue you a superbill, which is the form that you can submit to your insurance company for reimbursement.
